Forma Viva will refund, exchange or supply a credit note to the full purchase price of any item purchased within fourteen (14) days; subject to the following terms and conditions.

• The item purchased must be returned in its original packaging and with all of the required documents issued upon purchase such as certifications and warranties.
• The item must be in its original and saleable condition.
• The original proof of purchase must be supplied.
• Forma Viva reserves the right to hold the item for a reasonable amount of time, without a refund or exchange in order to authenticate that the item returned is the original item sold, i.e. has not been altered, and that there is no evidence of mishandling or abuse. If such evidence exists no refund or exchange can be given.
• Forma Viva will only refund in the same tender as the original purchase. If the transaction was by credit card or debit card the original card must be presented.
• Refunds or exchanges will not be given for special orders, items purchased on sale or items that have been altered.
• Should a layaway be cancelled, the purchaser will forfeit 10% of the purchase price unless it is transferred to the purchase of another item sold by Forma Viva.
